In 1973, American physician  Dr. Kenneth Lyons Jones first described Foetal Alcohol Syndrome, establishing that alcohol consumption during pregnancy — even in moderate amounts — could cause irreversible damage to a developing foetus. Growth deficiencies, fragile skeletal structure, distinctive facial features, developmental delays, poor impulse control, emotional volatility, and lifelong immaturity became recognised hallmarks of the condition.

Princess Margaret was born at a time when medical consensus held that drinking during pregnancy was harmless. Her mother, then Lady Elizabeth Bowes-Lyon, came from the hard-drinking Scottish aristocratic family that had occupied Glamis Castle for centuries. Her brothers were already famous for their capacity for alcohol, and Elizabeth herself would later become known, as Queen Mother, for beginning her daily intake by 11 a.m. During her first pregnancy in 1926 she suffered severe morning sickness and abstained, but four years later, while carrying Margaret, she faced no such restriction and drank freely — entirely in line with contemporary medical advice.

The consequences, Secrest argues, were lifelong and profound.

The Physical and Emotional Evidence

Margaret famously stopped growing at around five feet tall and maintained a delicate, almost childlike frame throughout her life. She aged quickly, suffered repeated illnesses, and had notoriously fragile bones. Contemporaries often described sudden rages, stubborn outbursts, poor judgement in relationships, and a recurring impulse to “run away” from responsibility — all classic behavioural markers of foetal alcohol damage.

Despite high intelligence and considerable charm, she struggled with maturity of judgement, was prone to tantrums, and repeatedly made choices that damaged her reputation and happiness. What the public saw as wilful self-sabotage or royal entitlement can, in this new light, be understood as the predictable outcome of neurological impairment that no one at the time recognised or knew how to manage.

Not Speculation — A Physician’s Assessment

Secrest is careful to emphasise that the FAS connection is not her personal theory. Dr Kenneth Lyons Jones, the doctor who originally defined the syndrome and who is still alive, personally reviewed Princess Margaret’s documented medical and behavioural history and concluded that she exhibited clear signs consistent with the condition. The diagnosis belongs to the physician who discovered FAS, not to the biographer.

A Life That Suddenly Makes Sense

Viewed through this lens, Margaret’s story shifts from one of scandal and privilege to one of quiet tragedy. Expected from birth to embody royal perfection in an era that demanded flawless public behaviour, she was instead born with a hidden disability that made such perfection neurologically impossible. The monarchy, with its rigid protocols and unforgiving scrutiny, was perhaps the worst possible environment for someone with impaired impulse control and emotional regulation.

A Call for Compassion

Ms. Meryle Secrest, recipient of the 2006  National Humanities Medal from President George W. Bush, and author of acclaimed biographies of figures ranging from Frank Lloyd Wright to Leonard Bernstein, describes this book as the most courageous of her long career.
